Introduction: Penetrating skull injuries in pediatrics (TPC) occupy a special place due to their rarity. The aim is provide an overview of the current evidence on decompressive craniectomy children, describe indications and detail complications. Reporte case: We present case 12-year-old boy who suffered TPC by projectile that perforated his head, causing multiple brain injuries, underwent early bilateral craniectomy, presenting complications neurological disabilities typical injury surgery, with reserved evolution prognosis. Conclusions: Pediatric penetrating should be individualized management, type disease, experience treatment team, decision always consensual front benefits risks short long term.
